Responsibilities:
- In addition to these tasks, employees may at times be asked to carry out other reasonable duties as required by the service or line manager;
- Be competent and confident in managing people with long term health conditions;
- Maintain client and business confidentiality at all times;
- To support service users in the CPA process and to ensure the effective and efficient use of all home resources;
- Behave in a professional and discreet manner at all times and in line with the Nursing and Midwifery Council (NMC) Code;
- Order supplies, maintain stock, ensure hygiene and cleanliness of equipment and environmen and safeguard medication;
- Support, manage and guide support workers on a daily basis;
- To utilise effective communication skills with both service users and staff within the organisation;
- Recognise and work within own competence level;
- Maintain confidentiality of documents, knowledge of residents and information obtained as pa of the role;
- Continual personal development to develop oneself and the role;
- Maintain an awareness of the Health and Safety requirements;
- Proactively identify, diagnose and manage treatment plans for service users (as appropriate);
- Assess, diagnose, plan, implement and evaluate interventions/treatments for service users with complex needs.
Requirements:
- Strong interpersonal skills (listening, verbal and written communication and IT skills;
- Planning and co-ordination of treatment packages and ability to prioritise and organise workload to meet targets and respond to unforeseen events;
- General nurse training/registration and 1st level.
