Our company is looking for a Care Coordinator to join our team.
Responsibilities:
- Taking new Service Users referrals from social workers and private Service Users;
- Answering and monitoring incoming telephone calls;
- Building strong relationships with the care team, patients and families;
- Providing excellent customer service at all times;
- Liaising with families and other healthcare professionals to ensure the quality of care we provide is the best in the marketplace;
- Accurately maintaining paperwork, including care plans, person centered plans, service user data check list, risk assessments and support plans;
- Setting up of new packages, including files and daily reporting;
- Implementing and creating paper work for the above packages;
- Allocating the right carer to the right customer at the right time;
- Supporting the Care Manager with business development;
- Recruiting of the next generation of Helping Hands Care Assistants;
- To work closely with the line manager and other colleagues to plan for ongoing recruitment and resource requirements in a proactive manner;
- To take responsibility for ensuring that all calls are covered in all instances, providing care personally if required;
- Inform the Registered Manager with KPI data to complete branch performance reports on a weekly basis;
- Manage customer and carer queries and action effectively and efficiently.
Requirements:
- Must have previous experience as a Care Coordinator in a Domiciliary Care Setting;
- QCF Diploma Diploma Level 3;
- Full driving licence and access to own vehicle is essential;
- Must have a UK Driving Licence with own transport;
- Access to an Employee Assistance programme;
- Must have a Full UK driving licence and own transport;
- Self-motivated and committed to the role;
- Able to work autonomously and in a team effectively;
- Able to work under pressure and to deadlines;
- Effective communication and listening skills;
- A professional but fun and social team environment in our Bolton branch;
