Community Mental Health Practitioner Functional Intensive Response and Support Team The Crystal Centre Part Time Hours 22.5 hrs per week.

As part of the transformation agenda a new service is being formed to provide support to Older Adults, with Frailty and Functional Mental Health needs.

As part of this developing service you will help support older adults with functional mental health issues to meet increased needs / rising risk to support admission avoidance and provide home treatment with a Home First approach to support. You will work with a new and growing team, the existing community services and the established Dementia Admission avoidance services across Mid and South Essex
Registered Mental health Nurse, or Occupational Therapist.
Additional Professional Qualifications in Older adult Mental Health /Dementia or Experience
Evidence of personal and professional development in contemporary mental health practice and dementia care
Non-medical Independent prescriber
Understanding of the Care Act
Good understanding of national policy and the implications for good mental health
Experience in the use of risk assessment tools
Use of screening tools in dementia
