Practice Nurse (Primary Care Nursing)

The Practice Nurse will provide:
assessment, treatment, screening and health promotion services and advice working in accordance with the NMC Code of Conduct and within their scope of practice.
nursing services to patients independently or in participation with Nurse/GP colleagues working to agreed protocols and guidelines.
Provide clinical services as designated by the Nurse Manager/Clinical Governance Lead and Provision of practice Nursing duties
Work with other health care professionals to monitor, manage and treat long-term conditions, in line with national and local policies and practice needs.
Support patients to adopt health promotion strategies that promote them to live healthily, and apply principles of self-care
Support and manage health needs of women presenting for family planning and cervical cytology consultations.
Participate in vaccination and immunization programmes for both adults and children.
Undertake immunisations, injections and vaccinations
Management of medical/nursing supplies, including stock control and expiry checks.
Maintenance of treatment room and clinical equipment including testing and recording where so defined and adherence to infection control procedures.
Maintain accurate comprehensive records of all consultations and treatments in the patient record and completion of all necessary documentation associated with patient health care.

RN level 1
Experience of working in Primary care nursing team
Immunisations child and adult
Travel vaccinations
Experience of working in Primary Care
Knowledge of the management of patients with long-term conditions
RGN with current PIN number
Practice Nurse experience
Membership of a professional body e.Post-basic Nursing qualification
Professional knowledge acquired through training to diploma/degree level
Appropriate training /experience in chronic disease management and other training as identified by their manager
Knowledge of accountability of own role in a nurse-led service
Knowledge of health promotion strategies
Knowledge of patient group directions and associated policy
An effective team player-flexible, adaptable and versatile.
Flexible approach to working hours
Knowledge and understanding of the Primary care sector
