What you will do:
Independently hold, prioritise, and manage a diverse caseload with the support of long-arm advice and supervision.

Engage in hospital discharges, rapid response, and preventative measures for admissions.
Employ holistic assessment techniques incorporating pressure management, nutrition, and hydration.
Drive across different hubs in the area, requiring access to your own car with business insurance.
Provide guidance and leadership to your team, handling complex situations and working collaboratively with multidisciplinary teams.
