Consultant in Renal Medicine

The Specialty Jersey General Hospital has 219 beds including an intensive care unit,. Renal medicine provides a comprehensive service for all aspects of kidney disease for adult patients with tertiary level support from Portsmouth with vascular access surgeries and transplant surgery supported by Cambridge University Hospital. Paediatric renal care delivered by University Hospital Southampton (UHS). The renal unit currently manages 35 haemodialysis patients and 65 individuals with functioning renal transplants. It operates an 18-station dialysis unit, including six cubicles. Nurses play a critical role, delivering acute and chronic dialysis, patient education, anaemia management, and post-transplant care. According to Jersey's national registry, approximately 2,720 residents (3% of the population) have CKD stages 35, with a mean age of 77 years. This prevalence is notably lower than the UK average of 6.1% among adults. Management Structure Health and Care Jersey is a directorate of the Government of Jersey that is responsible for the delivery of specialist health care and the running of the general hospital. The directorate is led by a Chief Officer, Tom Walker who leads an executive team including a Director of Clinical Service, Claire Thompson and a Medical Director, Simon West who is also Medical Responsible Officer/Suitable person with the GMC for appraisal and revalidation. Health and Care Jersey is organised as a series of care groups each with leadership team lead by a chief of service as detailed below. This post sits within the medical care group. Medical Services Care Group The Renal Department sits within the Medical Services Care Group which comprise the following range of services, a full structure chart of the care group can be found in the appendix: Cardiology Diabetes Emergency Admissions Unit (EAU) Emergency Department Haematology Medical Day Unit Medical Outpatients Medical Wards - Plemont, Corbiere and Rozel Wards Neurology Oncology Palliative Medicine Pathology Rehabilitation Ward Renal Medicine Respiratory Rheumatology Medicine in Jersey is fully age and specialty integrated. There are 4 medical wards, Plemont, Rozel, Bartlett and Corbiere. A 5-bedded Enhanced Care Area is sited on AAU and there is a separate intensive care unit led by intensivists. There is also an Ambulatory Emergency Care Unit led by the Acute Medicine Department. There is a 25 bed Acute Assessment Unit. SAS doctors are responsible for the day-to-day management of their patients on the wards with the help of clinical fellows, FY1 or FY2 doctor whom they closely supervise. On call, the SAS doctor is responsible for seeing patients referred by the Emergency Department and reviewing patients admitted to the AAU by GPs in conjunction with the Acute Medicine team. A core responsibility of the Medicine Care Group is the provision of a safe acute medical service. The Care Group always provides named Consultant supervision for non-Consultants and ensures that prompt review and treatment is provided for acute medical emergencies referred by General Practitioners or the Emergency Department. Consultants are responsible for producing and updating guidelines for the management of acute medical emergencies in their specialist area. Care Group Statistics There are 83 medical beds. 12-20 acute medical admissions on average during each 24-hour period on call within a range of approximately 6-24. There are approximately 4,500 medical admissions annually. In addition to outpatient clinics and 4 general medical wards containing 83 beds, the Medical Service Care Group is involved in the management of several specialist facilities. These include: 25 bedded Acute Assessment Unit (AAU) 5 bedded Enhanced Care Area (ECA) 6 chair Ambulatory Emergency Care (AEC) A well-equipped Clinical Investigations Department providing echocardiography, exercise ECG, respiratory assessment and GI physiology. An endoscopy suite providing a full range of endoscopy and bronchoscopy services. Diabetes Centre providing outpatient care for the diabetic population of Jersey, including retinal photography and specialist nursing support. Renal/Dialysis unit, providing emergency, acute and longer-term renal support as well as management of transplanted patients and assessment and treatment of renal conditions.6 bedded isolation area for infected or neutropenic patients. Registration It is a legal requirement for all doctors to be on the General Medical Councils Specialist Register before being able to take up a Consultant post. Specialty trainees are able to apply for a Consultant appointment provided the expected date of the award of their Certificate of Completion of Training (CCT) (or recognised equivalent if outside the UK) falls no more than 6 months after the interview date for the Consultant position and candidates will be asked to provide proof of this prior to shortlisting.
