We are looking to recruit a caring and motivated Practice Nurse who is committed to patient care and willing to further develop our dedicated nursing team. We are ideally looking for a Nurse with a very strong background in Diabetes care and General Practice Nursing. The successful candidate will work collaboratively with our other Practice Nurses and HCA to treat our patients, maintain systems including infection control to meet CQC requirements and influence and implement policy and change in the Practice. This is a part-time position of 34 hours per week, across four days. Salary is competitive, depending on experience, with access to NHS pension scheme. Main duties of the job

To promote the mental, physical and emotional health of the Practice population
To assess the health needs of the Practice population
To provide and maintain a high standard of care for the Practice population
To be an effective member of the Primary Healthcare Team and to link with other organisations and disciplines in the development of services DIMENSIONS

To provide treatment room duties in line with competency e.g. Suture/clip removal, Basic & complex wound care, ECG. Phlebotomy, Urinalysis
To give vaccination as appropriate under patient group directions and to be responsible for the ordering of vaccines and their correct storage
To assist with monitoring and support of the management of patients with long-term conditions / to chronic diseases, using a holistic patient centred approach
To assist with the cervical screening programme
To administer prescribed drug therapy and ensure safe custody of drugs
To assist with Warfarin monitoring
To assist in the provision of minor surgery, coil and implant fittings
To assist with the co-ordination of Infection Control policy and audit.
To Chaperone patients when required
To provide training and supervision to junior members of nursing staff
To ensure confidentiality on all matters relating to patients and information obtained during the course of employment
To participate in the Practice/Trust/s staff development and performance management and supervision systems OTHER DUTIES
To undertake such other duties as may be reasonably required from time to time as are consistent with the responsibility and scale of the post
