What you will do:

* Manage a defined caseload of individuals with severe and enduring mental illness, delivering assertive outreach interventions within community settings.
* Undertake comprehensive mental health, risk and physical health assessments to inform recovery-focused care planning.
* Act as Care Coordinator, ensuring effective delivery and review of care plans in line with CPA and national mental health guidance.
* Develop and maintain engagement strategies for service users who may be reluctant to access mainstream mental health services.
* Complete risk assessments relating to self-harm, suicide, safeguarding, vulnerability and risk to others, implementing appropriate management plans.

Qualifications:

* Registered Mental Health Nurse (RMN) qualification.
* Current registration with the Nursing and Midwifery Council (NMC).
* Evidence of ongoing professional development relevant to community mental health practice.
* Mentorship, Practice Assessor or relevant mental health qualification desirable.

Experience:

* Post-registration experience within Community Mental Health Teams, Assertive Outreach Services, Crisis Teams or Recovery Services.
* Experience supporting individuals with psychosis, schizophrenia, bipolar disorder and complex mental health presentations.
* Experience managing high-risk and difficult-to-engage service users within community environments.
