Job summary
This is a part time permanent Band 6 Community Psychiatric Nurse position within the Older People Care Unit - Intensive Home Treatment Team in Mansfield and Ashfield.

The team supports individuals aged 65 and over who are experiencing a range of mental health conditions, including dementia, schizophrenia, bipolar disorder, personality disorder, anxiety, and depression.

IHTT aims to give people the choice and ability to live as independently as possible, based on the belief that individuals experiencing mental health difficulties are entitled to the highest quality of care.

Our service is delivered in people's own homes, with a strong focus on preventing unnecessary admission to hospital or residential care. We provide intensive support for up to 8 weeks in the patient's home, helping to optimise mental health, promote recovery, and maintain independence.

As part of our dynamic and fast-paced multi-disciplinary team, you will play a key role in delivering high-quality, person-centred care while contributing to innovative ways of working.

Our team brings together a wide range of professionals, including Consultants, Psychologists, Nurses, Occupational Therapists, Physiotherapists and Support Workers, who collaborate to provide holistic and specialist care for older people with complex, high-risk mental health needs.

Main duties of the job
You will act as a care coordinator, managing a caseload of patients and providing high-quality, person-centred care. Your role will include supporting the team's daily duty function, responding to patient and carer enquiries, and conducting initial assessments for new referrals into the service.

The Intensive Home Treatment Team operates 7 days a week, from 8:00 AM to 8:00 PM, including weekends and bank holidays, and you will be required to work flexibly within these hours as part of the rota.

You will work collaboratively within a multi-disciplinary team, contributing to care planning and clinical discussions to ensure safe and effective care. Accurate and timely documentation is essential, and you will be responsible for maintaining comprehensive clinical records, as well as reporting incidents and safeguarding concerns in line with Trust policy.

Your approach will be holistic and recovery-focused, involving families and carers wherever appropriate to support positive outcomes. You will also work closely with colleagues to review and manage the overall caseload and contribute to ongoing service development and improvement.

Job responsibilities
  • Provide intensive, community-based mental health support for older adults (65+) with complex conditions including dementia and other mental health diagnoses helping them stay in their own homes and avoiding unnecessary hospital or residential admission.
  • Conduct specialist assessments and deliver tailored interventions, all rooted in the Care Programme Approach for improved, personalised patient care.
  • Carry out follow-up assessments within 72 hours of patient discharge from inpatient settings.
  • Respond urgently: Patients receive an assessment at home within 1248 hours, with support typically lasting up to 8 weeks.
  • Provide clinical leadership, including education, supervision, and support for junior staff and students.
  • Triage of referrals, provision of initial assessments and one-to-one interventions with service users.
  • Devise and implement treatment plans for service users.
  • Demonstrate the application of specialist skills and knowledge in order to maintain professional competence and fitness to practice as a nurse practitioner.
  • Establish and maintain robust communication networks with clients, carers, team members, other health care and social care workers within urgent care and the community
  • Support discharge and onward referrals
Please note applicants will be required to pay for their DBS check. Costs are deducted from salary following appointment. The cost of the DBS application is £26.40 (standard) or £54.40 (enhanced), this cost will be deducted from your salary over the first 2 months of employment.

You are encouraged to enrol for the DBS Update Service. An annual fee of £16 per year applies.

Please note that this post does not meet the pay level required for a Skilled worker visa.

Successful applicants with no prior NHS experience would normally be placed at the bottom of the band in line with Agenda for Change. This salary is below the minimum salary required for sponsorship for a Skilled Worker / Health & Care visa. In these circumstances the Trust would not, therefore, be able to sponsor for a Skilled Worker / Health & Care visa.

  • Professional Qualification (Mental Health/Learning Disabilities/General) to degree level or equivalent
  • Maintains current registration with the NMC or equivalent registration.
