What you will do:

* Conduct comprehensive assessments of individuals struggling with potential dementia, providing timely diagnoses and treatment planning.
* Develop personalised care plans for patients, coordinating with caregivers and family members to ensure holistic support.
* Offer post-diagnostic support and implement problem-solving strategies for patients and their carers.
* Liaise with allied professionals and refer patients to pertinent support services for continued management and support.
* Work closely with the Alzheimer's Society to facilitate ongoing research participation and community resource connection for patients.

Requirements:

* Degree or equivalent qualification in Mental Health Nursing.
* Proven experience as a Mental Health Nurse is essential.
* Current NMC Registration.
* Ability to work 9-5 on Fridays consistently, with flexibility for two other days among Monday, Wednesday, Tuesday, or Thursday.
