The Patient Safety and Quality Midwife will support the delivery of high-quality, safe and equitable maternity care by providing expert advice and operational leadership in all aspects of patient safety, clinical governance and risk management. The post holder will work collaboratively with clinical teams and managers to promote a proactive safety culture, ensuring that effective systems and processes are in place for the reporting, assessment, investigation, analysis and management of clinical and non-clinical incidents and risks.
The post holder will work alongside patient safety manager to lead the operational delivery of the Patient Safety Incident Response Framework (PSIRF) within Maternity and Neonatal Services, ensuring that incidents are managed in accordance with national guidance, investigations and learning responses are completed in a timely manner, and that learning is embedded into clinical practice to drive continuous quality improvement.
The post holder will have a knowledge of the operational contact for the Maternity and Newborn Safety Investigations (MNSI) programme and other national patient safety bodies. In the absence of the patient safety manger the post holder will provide specialist advice to support the reporting of the eligible cases to MNSI and follow on the reports and ensure the improvement plans are complete.
Main duties of the job
- Lead the daily review, triage and escalation of maternity and neonatal patient safety incidents, ensuring timely reporting and management in accordance with the Patient Safety Incident Response Framework (PSIRF), Trust policies and national reporting requirements, including Maternity and Newborn Safety Investigations (MNSI).
- Act as the specialist patient safety resource for maternity, providing expert advice and guidance to clinical teams, managers and senior leaders on the identification, management, investigation and learning from patient safety incidents, including Patient Safety Incident Investigations (PSIIs), learning responses and Duty of Candour requirements.
- Promote and embed the Trust's Patient Safety Incident Response Framework, fostering a just and learning culture that encourages timely incident reporting, reflective practice and continuous quality improvement.
- Coordinate the management of patient safety incidents from initial reporting through to investigation, action planning and closure, ensuring investigations are completed within agreed timescales and that learning is shared across multidisciplinary teams.

Detailed job description and main responsibilities
- Support clinical teams to identify contributory factors and system improvements arising from patient safety incidents, using quality improvement methodologies and human factors principles to strengthen patient safety and reduce recurrence.
- Maintain robust oversight of maternity incident reporting, ensuring the quality, completeness and accuracy of incident data and supporting the effective use of electronic risk management systems. This is for the purpose of monthly maternity governance meeting and relevant divisional meetings, and this includes gynaecology as directed by patient safety manager in her absence.
- Analyse incident data, identify emerging trends, themes and areas of concern, and prepare regular patient safety reports and dashboards for the Maternity Governance Committee, Divisional Governance structures and Trust Board, working collaboratively with Business Intelligence and data teams as required.
- Monitor the implementation and effectiveness of recommendations arising from patient safety investigations, MNSI reports, complaints, inquests and national safety alerts, ensuring learning is embedded into clinical practice and improvements are sustained.
- Ensure learning and outcomes are disseminated across all areas.

- Self-motivating and act on own initiative, able to work independently
