What You Will Do * Advanced Clinical Assessments: Provide triage and clinical assessments at the mental health Single Point of Access , ensuring smooth and efficient entry into crisis services.
* Interventions & Preventive Care: Implement brief interventions to avoid unnecessary hospital admissions or visits to A&E, promoting early intervention and support.
* Collaboration with Agencies: Build strong relationships with key partners such as police , ambulance services , and local agencies to enhance support networks and improve service delivery.
* Mental Health Awareness: Contribute to training and mental health awareness initiatives, particularly for acute hospital staff.
* Care Plan Development: Work closely with service users and their networks to create safe, compassionate, and evidence-based care plans .
