Practice Nurse

Job summary
The Practice Nurse is a key member of the primary care team, delivering high-quality, evidence-based nursing care in a general practice setting. Working with GPs, Nurse Practitioners, Nursing Associates, Healthcare Assistants, and other team members. The Practice Nurse plays a vital role in managing long-term health conditions, promoting health and supporting screening programmes.

The role involves autonomous clinical practice within NMC scope, including Long term condition management (e.g. diabetes, asthma, COPD), interpretation of pathology results, wound care, immunisations, cervical cytology, family planning, and lifestyle advice. The nurse also contributes to audits, quality improvement initiatives, and service development aligned with national and local healthcare priorities.

Strong clinical skills, experience of working in general practice, excellent communication and interpersonal abilities, and a commitment to continuous professional development and patient-centred care are essential.

Main duties of the job
The main duties of the role will be providing a patient facing nursing service to the practice population. The role is expected to include but not be limited to:
  • Long term condition management (including diabetes, respiratory, hypertension)
  • Women's health
  • Vaccination and immunisation programme (including seasonal vaccinations)

We are also fortunate to have an integrated pharmacy team comprising Clinical Pharmacists and Pharmacy Technicians, who contribute significantly to the safe and effective management of prescribing and medication use.

This robust clinical workforce is underpinned by a highly skilled and efficient administrative team that ensures the seamless operation of the practice. Together, our multidisciplinary team is committed to providing outstanding care and continually improving health outcomes for the communities we serve.

Job responsibilities
The following are the core responsibilities of this post but will depend on whether the successful candidate has undertaken the relevant training and competencies.
  • Act independently and collectively within the practice team
  • Support the management of long term conditions including Diabetes, Asthma, COPD, IHD and LD
  • Manage own clinical practice and that of other members of the nursing team to ensure safe effective care
  • Accept delegated responsibility and inherent accountability for the quality of own practice
  • Assess, plan and evaluate clinical nursing care to meet the needs of the individual and/or identified group
  • Assess, diagnose and treat specific conditions in accordance with agreed professional protocols and guidelines
  • Take the clinical lead in defined areas of nursing practice within the primary health care team.
  • Monitor own and teams work areas and practices and ensure they are free from hazard and conform to safety legislation.
  • Receive direct referral from health care professionals, other agencies and patients
  • Plan, manager and deliver programmes to improve health and disease in individuals or groups.

  • Significant experience as a practice nurse in primary care setting 2 years or more
