The Paediatric Diabetes Specialist Nursing Team works alongside Shropshire and Telford Hospitals NHS Trust to deliver high quality, safe and effective care for children / young people, their families and carers. It offers multi-disciplinary clinics with Consultant, Nurse, Dietician and Psychologist across Shropshire, Telford & Wrekin and Powys. The Paediatric Diabetes Specialist Nursing Team is supportive of flexible working arrangements and newly recruited staff are offered a comprehensive induction package. Main duties of the job
To assess, plan and implement and evaluate evidence-based nursing care for children and young people. To provide effective nursing expertise, specialist advice and support to the nursing team and the wider MDT to ensure high quality care is delivered across Shropshire, Telford and Wrekin. To work closely with the wider MDT to provide support and direction in the education and training of patients and their families to deliver the highest standards of clinical care. Promote a "can-do" attitude and determination to develop and sustain dynamic and responsive multidisciplinary/ multi agency community services within the county which delivers best practice. Manage a complex specialised caseload providing high quality clinical education, advice, information and support to CYP and their carers/ families on all aspects of their condition and its effect on their lives About us
