The successful candidate will work closely with individuals, families, carers, and partner agencies to undertake strengths-based reviews, reassess needs, and ensure care and support plans remain appropriate, proportionate, and outcome-focused. You will play a key role in delivering high-quality social work interventions, safeguarding adults at risk, and ensuring compliance with adult social care legislation and best practice standards.
What will your responsibilities be?
- Undertaking strengths-based reviews and reassessments for adults with care and support needs, ensuring support remains appropriate and outcome-focused.
- Assessing the needs of individuals, families, and carers using recognised assessment frameworks and evidence-based practice.
- Reviewing, updating, and evaluating Care and Support Plans to ensure they reflect changing needs, strengths, wishes, and outcomes.
- Working collaboratively with service users, carers, families, NHS colleagues, Housing services, Police, Education, voluntary sector organisations, and other partner agencies.
- Promoting independence, wellbeing, and positive risk-taking through person-centred social work practice.
- Undertaking safeguarding enquiries and interventions in accordance with relevant legislation and safeguarding procedures.
- Responding to crisis situations, ensuring immediate protection, risk reduction, and the promotion of positive outcomes.
- Completing Mental Capacity Assessments and contributing to Best Interest decision-making processes where required.
- Planning, implementing, monitoring, and evaluating social care interventions in line with legislative requirements and best practice guidance.
- Managing a varied caseload and prioritising work effectively to meet service demands and project timescales.
- Maintaining accurate, timely, and high-quality case recordings, reviews, assessments, and reports.
- Working autonomously whilst maintaining professional accountability and contributing positively to team objectives and service improvements.
