What you will do

* Deliver high-quality community-based physiotherapy interventions within patients' homes.
* Work collaboratively within an integrated health and social care service alongside Occupational Therapists and support staff.
* Apply your rehabilitation and orthopaedic experience to support patient recovery and maximise independence.
* Assess, recommend, and provide minor equipment to support daily living and mobility needs.
* Conduct holistic assessments to identify and address wider health and wellbeing needs.
* Complete falls risk assessments and implement appropriate prevention and management plans.
* Develop and review personalised treatment plans to achieve positive patient outcomes.
* Maintain accurate clinical records and documentation in line with professional standards.
* Promote independence, functional mobility, and quality of life through evidence-based interventions.

Requirements

* HCPC Registration: Current registration as a Physiotherapist with the Health and Care Professions Council.
* Driving Licence: Full UK driving licence essential for community visits.
* Community Experience: Previous experience within community physiotherapy services is desirable.
* Rehabilitation Expertise: Experience in rehabilitation and orthopaedic physiotherapy.
* Assessment Skills: Strong clinical assessment and treatment planning abilities.
* Communication Skills: Excellent interpersonal skills with the ability to work effectively with patients, families, and multidisciplinary teams.
