This ongoing full-time locum role offers an excellent hourly rate of £33.73 and provides the opportunity to play a vital role in supporting adults through safe and effective hospital discharge. Working within a multidisciplinary team, you will help individuals regain independence, coordinate appropriate support, and ensure seamless transitions from hospital to home. * Complete Care Act 2014 assessments, reviews, and strengths-based support planning for adults with a range of complex needs.
* Undertake Mental Capacity Act (MCA) assessments and contribute to Best Interests decision-making where appropriate.
* Manage a complex caseload, ensuring effective care coordination and timely intervention.
* Promote independence by identifying community resources, support services, and preventative interventions that reduce the need for long-term care.
* Lead and contribute to safeguarding enquiries, ensuring vulnerable adults are protected while promoting choice and autonomy.
* Provide professional advice and guidance to colleagues, NHS partners, independent providers, service users, carers, and other stakeholders.
* Maintain accurate, comprehensive, and timely case records in accordance with organisational policies and statutory requirements.
* Participate in multidisciplinary meetings, discharge planning meetings, case conferences, and service development initiatives.
* Engage in regular supervision, reflective practice, and continuing professional development to enhance service delivery and professional practice. Requirements: * A recognised Social Work qualification.
* Current registration with Social Work England .
* Previous experience within Adult Social Care , preferably in hospital discharge, integrated hospital teams, or community services.
* Sound knowledge of the Care Act 2014 , Mental Capacity Act 2005 , safeguarding legislation, and relevant adult social care frameworks.
* Excellent assessment, care planning, communication, and partnership working skills.
* Ability to work effectively within a fast-paced multidisciplinary environment.
