Main duties of the job
The Nurse Practitioner will deliver advanced, autonomous and person-centred care within a 24/7 integrated triage and care coordination service. The role spans both in-hours and out-of-hours provision and operates across three key workstreams: Oncology and Haematology Helpline; Powys Care Coordination Centre (CCC), including the Single Point of Access (SPA); and Integrated Urgent Care (IUC) services for Powys.

The postholder will deliver expert clinical assessment, care planning, and make independent clinical decisions to ensure the best possible outcomes for patients and referring clinicians. Working within an extended scope of practice, the Nurse Practitioner will act as a senior clinical anchor across acute, primary, community and voluntary sector services, with a particular focus on supporting people living with cancer, individuals with complex needs, and those requiring urgent care navigation.

Shropdoc is a not-for-profit company established in 1996. We provide 24/7 Oncology and Haematology telephone triage service for patients receiving treatment from the Royal Shrewsbury and Telford Hospitals, and also across Lancashire, in addition to providing an urgent primary care services for patients across Powys.

KEY TASKS AND RESPONSIBILITIES
The Nurse Practitioner will provide advanced, autonomous, and person-centred care within a 24/7 integrated triage and coordination service. CLINICAL AND PROFESSIONAL RESPONSIBILITIES

• Deliver safe and effective triage and clinical assessment services via telephone and digital platforms both in-hours and out-of-hours, accurately collating information using electronic clinical systems and determining the most appropriate care response or escalation.
• Independently assess, interpret, and synthesise clinical information from a range of sources (including directly from patients or via call handlers) to make professionally autonomous decisions, avoiding unnecessary onward referral where safe to do so.
• Provide an evidence-based, nurse-led Oncology and Haematology Helpline managing chemotherapy-related toxicities, treatment side effects, and lifethreatening symptoms, with a strong emphasis on holistic support.
• Act as a Care Coordination Centre clinician, liaising with referring GPs, health and social care teams, and third-party providers to agree and implement care plans, ensuring safe and timely navigation through complex care pathways. This is for both local DGHs and Northwest area OOH for Oncology Patients.
• Provide health promotion, self-management education, and personalised care planning, working in partnership with patients and carers to support autonomy, wellbeing, and prevention of deterioration.
• Support clinical safeguarding responsibilities by identifying and escalating concerns in line with organisational policy, and by placing vulnerable adults or children at the centre of all risk-based decisions.
• Document all patient contact, interventions, and clinical risk decisions accurately using the appropriate electronic systems. Ensure adherence to professional, legal, and organisational documentation standards.
• Always uphold professional standards and ethical practice, maintaining registration, engaging in revalidation processes, and working within the scope of practice defined by the NMC or other relevant regulatory body.
• Maintain and promote dignity, respect, and inclusivity for all patients and carers, particularly addressing cultural, language, age, disability, or other barriers to health equity.
• Participate in and receive regular clinical supervision and mentorship to support safe practice, reflective development, and emotional resilience in high-pressure environments.
• Communicate and collaborate effectively with other health professionals, ensuring smooth handovers, continuity of care, and coordinated multidisciplinary responses.

SERVICE IMPROVEMENT DEVELOPMENT AND IMPLEMENTATION

• Contribute to the development and continuous improvement of a 24/7 integrated unscheduled care service, proactively identifying system gaps, inefficiencies, or opportunities for innovation.
• Participate in the creation and review of clinical protocols, care pathways, and operating procedures to align service delivery with evidence-based practice and local population needs.
• Lead or contribute to service audits, quality improvement projects, and research activity designed to evaluate and enhance patient outcomes, clinical safety, and staff performance.
• Represent the organisation with professionalism and act as a clinical ambassador in collaborative forums, partnership meetings, and when liaising with external providers.
• Promote safe working environments by complying with all statutory Health & Safety, fire safety, infection prevention, and clinical incident reporting policies. Report any adverse clinical events with recommendations for service learning and improvement.
• Take responsibility for clinical equipment care, ensuring safe use, calibration, and prompt reporting of faults or risks.
• Support the ongoing development of the workforce, mentoring junior or newly appointed colleagues and contributing to the organisational learning culture through formal and informal training opportunities.
• Maintain awareness of national and local developments in oncology, urgent care, and digital triage innovation, integrating relevant updates into local practice and sharing insights with the wider team.

• Computer literate - able to use various forms of media and IT systems. Ability to type patient notes when on the phone.
• Broad understanding of managing patients in a primary care or emergency setting.
• Understanding of equality and diversity issues in a healthcare delivery setting.

Essential

• Ability to attend work on a regular basis and meet the requirements of the role with any reasonable adjustments which have been notified and arrangements made under the Equality Act.
• Flexibility in working in a rostered environment including night shifts, bank holidays and weekend working.
• To work in the call centre for every shift. Remote working to be utilised only in times of escalation to assist Shropdoc in meeting clinical demand.
