Continuing Healthcare Nurse Assessor (Band 6)
Location: Lancashire and South Cumbria (Blackpool, Preston, Blackburn, Hyndburn, Wyre, West Lancashire & South Lakeland)
Pay rate: £21.57 per hour
Hours: Full-time (37.5 hours, Monday to Friday) or part-time (24+ hours considered)

What youll be doing
- Carry out assessments for NHS continuing healthcare and funded nursing care
- Gather and analyse clinical evidence to support Decision Support Tools (DSTs)
- Work with multidisciplinary teams to make eligibility recommendations
- Present cases for panel decision-making
- Maintain accurate, contemporaneous patient records in line with NMC standards
- Build effective working relationships with NHS, local authority and partner organisations
- Support consistent, high-quality assessment practice across services

Essential:
- Registered nurse with valid NMC registration
- Experience working in continuing healthcare (ICB or CCG)
- Understanding of NHS Continuing Healthcare Framework and guidance
- Experience supporting patients with complex needs
- Strong assessment, communication and organisational skills
- Degree-level qualification (or equivalent experience)
- Experience working across health and social care organisations
- Knowledge of safeguarding and relevant legislation

- 6 month ICB experience within the last 3 years
- Fully conversant with the assessment process and need to assess, plan, implement, and evaluate the needs of clients.
- Carrying out complex nursing/healthcare assessments for clients with health and social care needs.
- Care planning for clients with complex health and social care needs.
- Assembling specialist packages of care which are tailored to the needs of individual clients.
- Awareness of the National Framework for NHS Continuing Healthcare and NHS funded Nursing Care.
- Knowledge and understanding of NHS and Social Care Services structures and Independent Sector Organisations.
- Knowledge of the Care Act 2014.
- Vulnerable adults legislation.
