Main duties of the job
- To provide a highly specialised clinical pharmacy service to the Specialist Medicine Service department as part of the multidisciplinary team which includes the following:
- To provide clinical pharmacist ward cover as per the rota.
- Optimisation of medication for the individual patient ensuring medicines reconciliation, appropriate clinical screening as well as discharge support via drug listing.
- To provide clinical advice and care to specialist medicine inpatients when required.
- Provision of homecare prescription management
- Contribute to virtual biologic clinics and other multi-disciplinary meetings
- Development of evidence based prescribing guidelines
- To manage a Pharmacist-led specialist medicine Clinic:
- To prescribe medication within scope of practice
- To develop external links, local primary care organisations and any other relevant local and national specialist interest groups.
- To promote high-quality, cost-effective prescribing for patients
- To provide the Associate Director of Pharmacy and Clinical Services Manager with details of output, performance and activity at specified time intervals in an agreed format.
- To lead on the implementation and continuation of biosimilar switch schemes and ensure that patients are switched and within the required timeframe.
