Part Time -- 30 Hours per Week We are delighted to offer an exciting opportunity for a Band 6 Advanced Kidney Care (AKC) Clinical Nurse Specialist to join our dedicated and supportive renal team. We are looking for an exceptional nurse with substantial experience and a passion for delivering high-quality, patient-centred care. As an Advanced Kidney Care Clinical Nurse Specialist, you will work closely with the Lead AKC Nurse to manage a designated caseload of patients with chronic kidney disease (CKD). You will play a key role in empowering patients to make informed decisions about their treatment by providing both face-to-face and virtual education, delivering nurse-led clinics, and supporting patients as their kidney function declines. Responsibilities include:
- Managing a caseload of patients with advanced chronic kidney disease
- Providing specialist assessment, education & support for patients & their families.
- Facilitating informed decision-making regarding kidney replacement therapies including haemodialysis, peritoneal dialysis, kidney transplantation & conservative kidney management.
- Leading nurse-led clinics & conducting comprehensive patient assessments.
- Coordinating care across primary, secondary & tertiary services.
- Acting as a key worker & advocate for patients throughout their treatment pathway.
- Supporting advance care planning discussions as appropriate.
- Working collaboratively with nephrologists, palliative care teams, dietitians, pharmacists, psychologists, social workers & community healthcare professionals.
- Participating in service improvement, audit, research & quality initiatives.
- Providing education & support to nursing colleagues and the wider multidisciplinary team.
- Registered Adult Nurse with NMC registration.
- Excellent communication, counselling & interpersonal skills.
- Experience of complex patient assessment & care planning.
- Ability to work autonomously whilst contributing effectively within a multidisciplinary team.
- Evidence of continuing professional development.
- Strong organisational & caseload management skills.
- Commitment to delivering personalised, patient-centred care.
- Valid UK driving license and ability to drive to provide education & support to patients in the community
