Clinical Lead / Care Coordinator / Registered Nurse
Part-Time, 25 Hours Per Week, Monday to Friday
Lumen Healthcare is looking for an experienced and clinically confident Registered Nurse to join our growing team as a Clinical Lead / Care Coordinator.
Lumen Healthcare provides high-quality home care, live-in care, nursing and specialist complex care to adults across Birmingham and Solihull. Our services support individuals with a wide range of health and social care needs, including neurological conditions, acquired brain injury, spinal cord injury, learning disabilities, autism, dementia and complex physical health needs.
Our complex-care services may include tracheostomy care, invasive and non-invasive ventilation, suctioning, cough assist, oxygen therapy, enteral feeding, PEG care, catheter care, stoma care, complex bowel care, epilepsy management, rescue medication, wound care and other delegated or nurse-led clinical interventions.
We are looking for a Registered Nurse who can combine strong clinical judgement with excellent organisational skills and who will support the Registered Manager with the safe coordination, monitoring and development of our clinical services.
You will work 25 hours per week, Monday to Friday.
Your day-to-day at Lumen Healthcare
- Complete comprehensive clinical assessments for new and existing clients.
- Develop, review and update person-centred care plans, risk assessments and clinical management plans.
- Coordinate domiciliary, live-in and complex-care packages across Birmingham and Solihull.
- Assess whether staff have the appropriate skills and competencies before allocating them to individual care packages.
- Provide clinical oversight of Registered Nurses, Complex Care Assistants and Support Workers.
- Undertake clinical competency assessments for staff undertaking delegated healthcare activities.
- Monitor complex-care packages including tracheostomy, ventilation, enteral feeding, epilepsy, catheter, stoma and bowel-care services.
- Liaise with GPs, community nurses, hospital teams, specialist nurses, therapists, case managers, commissioners and other healthcare professionals.
- Participate in multidisciplinary meetings and clinical reviews.
- Monitor medication management and ensure safe practice in accordance with local policies and professional standards.
- Review incidents, accidents, medication errors, safeguarding concerns and clinical deterioration.
- Undertake clinical audits and identify areas for improvement.
- Monitor care records, electronic documentation and staff compliance.
- Complete spot checks and quality-assurance reviews.
- Support investigations into complaints, incidents and clinical concerns.
- Ensure appropriate escalation where a client’s condition changes.
- Support hospital discharge and transition-of-care planning.
- Ensure clinical equipment and specialist care requirements are clearly documented before commencement of a package.
- Maintain accurate and professional clinical records.
- Provide clinical advice and support to care staff.
- Support recruitment and clinical interviews for Registered Nurses and complex-care staff.
- Contribute to staff induction, training, supervision and competency development.
- Work closely with the Registered Manager to maintain safe, effective and person-centred services.
- Assist with preparation for CQC inspections and ongoing regulatory compliance.
- Participate in service-development and quality-improvement projects.
Essential Qualifications
- Registered Nurse qualification.
- Current unrestricted Nursing and Midwifery Council registration and PIN.
- Minimum of three years’ post-registration nursing experience.
- Significant experience within complex care, critical care, intensive care, respiratory care, community nursing or another relevant clinical environment.
- Evidence of continuing professional development.
- Current Basic Life Support training.
- Enhanced DBS, or willingness to undertake one.
- Full UK driving licence and access to a vehicle, where required to visit clients across Birmingham and Solihull.

- Experience developing and reviewing nursing care plans and risk assessments.
- Experience supervising or supporting healthcare staff.
- Strong understanding of medication management.
- Ability to identify deterioration and escalate appropriately.
- Excellent written clinical documentation.
- Ability to complete audits and use findings to improve quality.
- Good understanding of safeguarding adults.
- Good understanding of the Mental Capacity Act 2005 and consent.
- Strong understanding of professional accountability and the NMC Code.
- Ability to manage several care packages simultaneously.
- Excellent communication and multidisciplinary working skills.
- Strong organisational and time-management skills.
- Ability to work independently and make appropriate clinical decisions.
- Ability to provide constructive clinical leadership to nurses and care staff.
- Commitment to dignity, independence, choice and person-centred care.
Desirable Qualifications and Experience
- Previous Clinical Lead, Senior Nurse, Care Coordinator or Community Nurse experience.
- Experience within ICU, HDU or complex respiratory care.
- Tracheostomy competency.
- Invasive and non-invasive ventilation competency.
- Suction and cough-assist competency.
- PEG and enteral-feeding competency.
- Catheter and stoma-care competency.
- Complex bowel-care experience.
- Epilepsy and rescue-medication competency.
- Experience supporting clients with spinal cord injury, acquired brain injury or neurological conditions.
- Experience working with learning disabilities and autism.
- Experience completing staff competency assessments.
- Practice assessor or mentorship qualification.
- Experience using electronic care-planning and eMAR systems.
- Experience working within a CQC-regulated domiciliary or complex-care service.
