Senior Community Mental Health Nurse (Band 6)

We are seeking a motivated and experienced Senior Community Mental Health Nurse (Band 6) to join our dynamic Adult Community Mental Health Service in Telford and Wrekin. This is an exciting opportunity to play a key role in delivering high-quality, person-centred care to individuals experiencing mental health difficulties. You will manage a complex caseload, contribute to service development, and support colleagues through supervision and clinical leadership. Reporting Structure * Reports to: Operational Lead / Quality Lead * Accountable to: Service Manager / Quality and Governance Lead Key Responsibilities * Manage a complex caseload, including assessment, care planning, and evaluation of treatment plans * Undertake comprehensive clinical assessments to identify needs, risks, strengths, and goals * Develop and implement formulation-based care plans in collaboration with service users and carers * Deliver evidence-based nursing interventions aligned with agreed care plans * Provide clinical supervision and support to team members where appropriate * Participate in quality assurance activities, including audit, monitoring, and service improvement * Contribute to the development of clinical standards in line with current guidance and policy * Work flexibly across service functions based on skill set and service needs Working Relationships
