We are hiring a Care Assessment Coordinator to join our Care Assessment Team at Xyla Healthcare Services , a division dedicated to delivering high-quality assessment services that help improve outcomes for service users across the UK.
In this role, you will coordinate the end-to-end assessment process for Continuing Healthcare services, working closely with service users, families, clinicians, commissioners and other stakeholders. You'll play a vital role in ensuring assessments are delivered efficiently, supporting patient flow and helping people access the care and support they need.
In this role, you will:
Coordinate assessments, reviews and appeal meetings to agreed service levels and operational KPIs.
Liaise with service users, families, clinicians, Integrated Care Boards (ICBs) and other stakeholders to manage assessment bookings and progress cases.
Obtain and manage required documentation, including care home and GP records, within agreed timeframes.
Act as the central point of contact for your service area, providing updates and maintaining strong stakeholder relationships.
Prepare appeal documentation and accurately record meeting minutes to support high-quality service delivery.
