Job description/Key Responsibilities
- Personal care support, including medication reminders and personal hygiene.
- Meal and Fluid preparation
- Accessing the community, assisting clients with shopping and errands.
- Travel to and accompaniment to social and medical appointments.
- End of Life Care
- Support with discharge from hospital
- Dementia Care
- Sit in Care
- Domestic duties and housekeeping
- Care provision
Person Specification
- Good understanding of the care and support needs of the client
- Ability to use a person-centred approach to undertake the tasks detailed in the client’s care and support plan and in the least intrusive way
- Confident providing input into the care and support plans of clients by regularly feeding back to management
- Ability to use manual handling equipment safely and correctly.
- Good communication skills and ability to develop effective working relationships with clients.
- Listening skills and able to respect the needs of the client.
