Job summary
An exciting opportunity has arisen for a dynamic, enthusiastic pharmacist to join the anticoagulation and thrombosis MDT team. The position will provide a crucial role both in supporting inpatient work streams and optimising anticoagulation in primary care. You will work closely alongside Consultants and specialist nurses to deliver care to patients with bleeding and thrombotic disorders. Excellent communication and organisational skills are essential. Please note this is 12-month fixed term position to cover maternity leave of the current post holder. Main duties of the job

- This post includes split duties between the local Anticoagulation Optimisation Support Service (AOSS) to primary care and the clinical anticoagulation role within Oxford University Hospitals NHS Foundation Trust.
- As part of the AOSS to deliver on the pharmaceutical aspects of anticoagulation and thrombosis
- To demonstrate good clinical knowledge, accepting and managing case referrals for medicines optimisation
- To be responsible for the review of patients on warfarin therapy whose anticoagulation control is poor
- To unify and simplify anticoagulation guidelines within primary care
- To contribute to educational teaching and provide clinical support for GPs and pharmacists
- To provide an email and telephone advice support service to deal with complex anticoagulation enquiries from healthcare professionals within primary care
- To participate in anticoagulation clinics primarily for those patients on warfarin with poor control
- To be a member of the Cancer Team
- To support the Pharmacy Education and Training Team to ensure the competency of all pharmacy staff who are involved with the prescribing, supply and administration of anticoagulants
- To be an active member of the anticoagulation and thrombosis team and ensure continuous two-way exchange of information across the interface and with key stakeholders
- To work as part of the Trust's anticoagulation team (adults) for the pharmaceutical aspects of anticoagulation and pharmacological thromboprophylaxis
