Our Urgent Care Nursing Service is seeking to recruit Paramedics, Nurses and Nursing assistants to join our Urgent Care Nursing team to support the development and implementation of a new and exciting proposal to support people who have fallen in their own homes. These roles would respond to calls from the ambulance service, hospital emergency department or community services to support someone out of hours who has fallen but not injured themselves. The post holders would assess, or support the assessment, if appropriate assist to get the person up (with appropriate training and equipment) and if needed the Community Nursing Assistant would stay with the person to keep them safe until the morning when further assessment and support is available.

If there isn't a faller needing a response overnight the post holders will support our routine Urgent Care Nursing work which consists of visiting patients in their place of residence, home or care home, to provide urgent nursing care, primarily end of life care, catheter and wound care.

Main duties of the job
This element of our service will run from 20.00-08.00 and covers Plymouth, the South Hams and West Devon. Pool cars are provided for patient visits, training will also be provided and learning and development needs are positively supported. This is a brand-new area of service not currently established, the aim is to develop a swift response to avoid unnecessary hospital admission for people who have fallen but are not hurt and to keep them safe and well in their own home.

This post is designed to support the roles and functions of the team members in UCNS by providing professional nursing advice, shift leadership, treatment, and support. The service runs 24 hours a day and covers a 7-day service and managing the service in the absence of manager and acting as first on call for the team (if required).

The post holder is primarily responsible for the assessment of patients who have fallen at home, working alongside a health care assistant on each shift. The post holder will assess if patient is safe to be moved with the use of specialist equipment. Full risk assessment will be undertaken by post holder to support patient to remain at home and referral, if appropriate to supporting services.

This role may not be eligible for sponsorship under the Skilled Worker route.

Job responsibilities
5.1 Responsibility for People Management

• To provide leadership for Band 5, HCA and Admin, through supervision, line management, appraisal and facilitate them to achieve their competencies
• Be responsible for the day-to-day management of nursing team without direct supervision and carry out operational / leadership duties as appropriate
• Support, complete and monitor staff to ensure the submission of necessary forms, documentation, including IT data and forward as required by the manager / organisation are undertaken in a timely manner
• Be aware of pressures facing work colleagues and offer support and ensure they are aware of organisation support services available to them
• Lead and participate in team activities to develop and consolidate a cohesive and openness within the team
• Be responsible for maintaining lone working information for identified staff group and monitoring staff adherence to lone working policy and use of devices
• Have an ongoing responsibility and duty of care to patients, staff, self and others in all aspects of health, safety and risk
• Act as a role model promoting good performance and practice
• Reporting all incidents and near misses in line with organisational policy
• Inform manager of any ongoing nursing/ management issues affecting patient care or working capacity

5.2 Responsibility for financial and/or physical resources

• The post holder will use mobile IT and telephony, being responsible for the security and safety thereof.
• Will be responsible for the use of specialist equipment which will be carried in pool car.
• Will be given the use of sundry equipment and is the responsibility of the post holder to keep such equipment safe, maintained and cleaned
• The post holder will be given the use of mobile phone and lone working device for work purposes and be expected to abide by organisational policy

5.3 Responsibility for administration

• Manage and coordinate caseload. Triaging calls, prioritising patient clinical needs and delivering high standard of care
• Maintain accurate records, which are confidential, up to date and legible and demonstrate that all care given is documented. These records may be paper or IT based systems and must be maintained as specified in organisational policy, NMC guidelines and government directives
• The majority of records will be recorded using system 1 electronic system.
• Be required to use a computer, being responsible for the timely recording of patient activities for IT data collection
• Preserve confidentiality and comply with the data protection act, access to health records and consent for treatment

5.4 Responsibility for people who use our services

• Be responsible for the provision of planned and unplanned care to patients and their Carers across Plymouth, South Hams and West Devon geographical area working as part of a 24-hour District Nursing Service
• Be able to respond to urgent referrals for patients, carers and the multi-disciplinary team
• To triage and accept appropriate referrals directly from other disciplines, patients, carers via locality hub and palliative care line.
• Be responsible for assessing, diagnosing, planning, organising and reviewing complex packages of care including case conferences
• Provide holistic evidence based/best practice care to patients in accordance with National and Organisational approved policies/procedures and individual care plans
• Ensure privacy, dignity and human rights are respected for all patients and carers
• Undertake comprehensive risk assessments of all situations associated with the care patients in order to ensure nurses and the carers safety. Assessments might include the provision of equipment ensuring it is used safely and within the manufacturers guidance and reviewed as per Livewell Southwest policy
• Be responsible for overseeing and monitoring of equipment, ensuring maintenance and repair
• To report and records all incidents and near misses relating to health, safety, security, fire, physical violence, aggression and verbal abuse
• The post holder is required to comply with all infection control policies and guidance, attend relevant updates and report issues of concern to their immediate line manager (if no action or explanation received, then it is the individuals responsibility to escalate to Director of Operations or Chief executive office)
• Be conversant and adhere to the vulnerable adult and child protection procedures and Organisational polices

5.5 Responsibility for implementation of policy and/or service developments

• Identify community health needs and participate in new projects, research and audit
• Be involved with the development, implementation, and monitoring of standards
• Participate in policy development through professional forums and implement practice ensuring the clinical practice reflects national and local drivers
• Ensuring all relevant information regarding patients is recorded, reported and where necessary acted upon to communicate changes to the GP and other team members as appropriate
• Identify areas of practice / role development and enable and support staff to initiate change
• Implement, participate and facilitate Clinical Supervision as per Organisational Policy

Qualifications

• Registered Nurse with relevant registered experience
• Teaching skills
• Ability to frequently travel independently and in a timely manner between Livewell Southwest sites and community locations including areas not served by public transport.
