MDT Administrator & Pathway Tracker

Job overview

An exciting opportunity has arisen for an enthusiastic and self motivated individual to join the Diabetes and Endocrine team as a Pathway Tracker and MDT administrator.

We are seeking a well organised team player with excellent communication skills who thrives in a busy, challenging yet rewarding environment. 

 

In this role, you will be responsible for validating, amending, tracking and navigating 18 week patient pathways and improving data quality for all patients under the MODEL services as well as ensuring our patients are booked within local and national targets. 

 

This post also includes admin for the Adrenal MDT meetings including adding patients and ensuring all relevant information such as imaging; external letters and reports are gathered and available for the MDT meetings.

Main duties of the job

Working within a complex environment the post holder has responsibility for managing, tracking and navigating 18 week patient pathways and reporting at relevant meetings, ensuring that national and local targets are met.

Responsible for validating amending and improving data quality in relation to pathways from initial referral, through diagnostics to completion of patients episode.

Proactively ensuring the patient has timely appointments within their pathway in order to achieve the waiting time targets.

Liaising and co-coordinating with all colleagues involved in the patient’s management.

Managing the Adrenal MDT and ensuring that the correct patients are added to the list to be discussed.

Ensuring that all required information such as imaging and external reports are available and uploaded ready for the MDT discussion.

Sending outcomes from the MDT meeting to the relevant colleagues to action and following up to ensure these actions have been taken.

Due to Home Office immigration rules, a full time permanent vacancy cannot be filled by individuals on a Student visa. Therefore, please be advised that if you are a Student visa holder, we will not be able to offer you a full time permanent contract unless you have:

-        applied for a Graduate visa

-        or you will have successfully completed your course and have applied for a Graduate visa before the anticipated start date of your employment

Qualifications

Essential criteria

GCSE Maths & English or Equivalent

Customer Service/ Business Admin /ITQ or equivalent professional experience

Desirable criteria

Relevant training on RTT pathway management

Experience

Essential criteria

Experience of working within a healthcare setting

Prioritising, planning and organising own and others tasks.

Experience of dealing with difficult situations and confrontations

Data entry experience and documentation

Desirable criteria

Previous experience of working in NHS

Previous experience of working with medical records

Experience of working with EPIC hospital system

Experience of working with CHEQs

Experience of presenting data and information to various stakeholders or team members

Knowledge

Essential criteria

Confidentiality and data protection processes and procedures

Customer service

Good working knowledge of office procedures and all Microsoft office packages

Awareness of NHS targets and Trust priorities

Knowledge of RTT targets
