Job summary
We currently have opportunities within our City CMHT's, 1 substantive full time post and a part time substantive post and 2 fixed term Band 6 Community Mental Health Nurse post's open to applications from Registered Mental Health Nurses. The team is based in central Norwich and covers central based Gp practices. We are a recovery focussed, multi-disciplinary team providing an effective, supportive, and professional service for our Services users and Carers. You will be a qualified professional with experience of supporting people and their families, preferably in the community. You will demonstrate a strong understanding of your role as a Care Coordinator/Assessor and join our dedicated multidisciplinary team working closely with a range of partners to provide appropriate community interventions for those individuals referred to the team. The ideal candidate will have an interest in working within a busy and dynamic Community Mental Health Service. The post holders will undertake the planning/delivery of direct patient care with minimal supervision, ensuring continuity of practice and working within a MDT/multi-agency environment.

Within this role you will work with an established and supportive MDT. As with many services we currently use a blended model of virtual and face-to-face working which provides opportunities for flexibility. You will have knowledge, skills and experience of working across the range of complexity and severity in adult mental health. We will expect the post holder to work collaboratively and flexibly with others to undertake assessments, develop and share psychological formulations and develop treatment plans. You will be responsible for managing a caseload, assessing and planning treatment for your patients. Working with the patient to plan care and optimise treatment plans. - Managing a caseload and the provision of mental health care services to people with mental health care need.
- Provide planned care/risk assessment / screening for patients within their care plan and ongoing evaluation of patients' needs and identify suitability for potential discharge in conjunction with the MDT and via caseload management/supervision.
- Initiate and attend patient centred reviews, case conferences and other meetings as required.
- To advise the Responsible Clinician(s) and/or health and social care professional of any change in the patients' conditions and / or circumstances.
- To work within the MDT and to form working partnerships with other care providers.
- To ensure service users/carer(s) are enabled and empowered to make informed decisions about their care and delivery of that care, working with agencies from within and outside the MDT.

- Nursing degree or equivalent Mentoring / Mentorship (ENB988 or equivalent)
