Job Title: Occupational Therapist - Community Adaptations Team

Salary: Up to £28.40 per hour Occupational Therapist within the Community Adaptations Team in Birmingham, earning up to £28.40 per hour. What you will do: * Conduct community-based Occupational Therapy assessments within service users' homes. * Assess the need for specialist equipment, minor adaptations, and major adaptations to promote independence and safety. * Make recommendations to support individuals with physical health needs and functional difficulties. * Utilise your knowledge of manual handling principles to ensure safe care arrangements and practical support solutions. * Complete assessments for home adaptations and liaise with relevant professionals throughout the process. * Work collaboratively with Occupational Therapy colleagues and partner agencies to develop person-centred intervention plans. * Maintain accurate, timely, and comprehensive case records and assessment documentation. * Support service users through the adaptation process, ensuring recommendations meet identified needs and desired outcomes. * Attend Sparkbrook Health and Care Centre one day per week for administrative tasks, team meetings, and collaborative working. * Contribute to the delivery of positive outcomes by promoting independence, accessibility, and wellbeing within the community. Requirements: * Degree or equivalent qualification in Occupational Therapy. * Current HCPC registration. * Experience within community Occupational Therapy services. * Knowledge and experience of equipment provision, moving and handling, and home adaptations. * Strong assessment, communication, and problem-solving skills. * Full UK driving licence and access to a vehicle. * Ability to work independently while contributing effectively within a multidisciplinary team.
