Physical Activity & Wellbeing Practitioner

£14.11k per annum

Age UK Lincolnshire is expanding to meet the growing demand for our exceptional services, and we're looking for a dedicated Physical Activity & Wellbeing Practitioner to join our Communities in Motion Programme. Based in Lincoln with regular travel to Skegness , this is an exciting opportunity to empower older people to become more active, improve their confidence, and maintain their independence through tailored physical activity support.

At Age UK Lincolnshire, we focus on supporting older people to live well, stay independent and flourish in later life. Through one-to-one support, encouragement, and personalised activity plans, you'll help older people overcome barriers, build confidence and make positive changes that improve their health and wellbeing.

About the role

  • 20 hours per week
  • Fixed-term contract until July 2028
  • £14,106 per annum (£24,687 FTE)
  • Community-based role covering Lincoln and regular travel to Skegness
  • Full UK driving licence and access to your own vehicle is essential
  • Training provided

You will provide tailored support that includes:

  • Delivering one-to-one physical activity interventions for older adults participating in a 12-week programme
  • Assessing individual needs, abilities, goals and barriers to physical activity
  • Developing personalised activity plans that build confidence, strength, mobility and independence
  • Motivating and encouraging participants to become more physically active
  • Monitoring progress and adapting activities where required
  • Supporting, training and coordinating volunteer mentors involved in programme delivery
  • Managing participant referrals and onboarding processes
  • Maintaining accurate records and programme data
  • Supporting monitoring, evaluation and reporting to demonstrate programme outcomes and impact

About you

· Are you passionate about helping people improve their health and wellbeing?

· Would you like to support older adults to become more active, improve their confidence and live independently for longer?

The successful candidate will:

  • Have experience working with adults, health improvement programmes, community engagement or physical activity initiatives
  • Be able to motivate and build confidence in people who may be inactive or lacking confidence
  • Have a person-centred approach and excellent communication skills
  • Be organised, proactive and able to work independently and as part of a team
  • Have good IT skills and experience maintaining accurate records
  • Be approachable, empathetic and passionate about supporting older people

Experience of volunteer coordination, health coaching, exercise referral, rehabilitation, social care or a related field would be advantageous, although full training and support will be provided.
