Community Staff Nurse

Job summary
We are seeking a Registered Adult Nurse (NMC Adult/Level 1) who is passionate about delivering high-quality physical healthcare within the community. This role is for Adult Registered Nurses and is not suitable for Mental Health or Learning Disability nursing registrations unless applicants also hold current Adult Nursing registration. The service is seeking a Community Nurse who is keen to work and develop their skills within the community setting.

As a Community Staff Nurse, you will work within the Integrated Care Team, led by the Integrated Senior Clinical Lead Community Lead. You will be working alongside other Health Care Professionals. You will be an autonomous practitioner and have excellent communication and interpersonal skills.

You will work to a high professional standard, which will enable us, as a team, to provide high quality, person-centred care. The post holder will have responsibility for managing co-ordinating and delegating work ensuring referrals are dealt with according to clinical need, Holistic assessments and care plans are recorded in line with Trust and Professional record keeping standards and clinical care at all times meets acceptable standards of patient safety and quality outcomes.

Main duties of the job
Working within Professional guidelines and accountability, the post-holder will have responsibility for the management of a defined community caseload (via Nursing, Physiotherapy or Occupational Therapy interventions) for a group of GP Practices as part of the Integrated Care Team.

The post holder will be required to work with a high degree of autonomy and professional accountability. A primary aim of the role is to prevent unnecessary admission of patients to an acute hospital and support timely discharge into the community. The post holder will also have a responsibility for re-enablement, rehabilitation, health protection and promotion programmes that improve and reduce inequalities.

The post holder will be required to:

  • Person-Centred Care
  • Participate in the holistic assessment, implementation and evaluation of programmes of care and therapy for patients in the home, without direct supervision. This includes those with chronic disease, terminal illness who wish to die at home and complex care needs. Other settings include, residential homes, nursing homes, clinic's community centres and community hospitals.
  • Recognise and respond to the need for appropriate referral to other members of the Community Care Team or external organisations including appropriate escalation of potential Safeguarding risks.

Qualifications
Essential
  • BSc Nursing or RN currently registered with NMC. Practice assessor/supervisor. Post registration, specialist training relevant to speciality. Current (UK/EU) Driving Licence and access to own vehicle for work purposes is required.
Desirable
  • Access Course / Diploma in Health Care Studies. Post Registration Qualifications.
Experience
Essential
  • Own transport and driving license, in order to be able to travel

  • Previous Care Home Primary Care experience
