Job summary
An exciting opportunity has arisen for an experienced clinical pharmacist to join NELFTs developing Adult ADHD Service as an Advanced Clinical Pharmacist.

Working as a senior and autonomous member of the multidisciplinary team, the post holder will provide expert pharmaceutical care throughout the adult ADHD pathway. This will include delivering non-medical prescribing clinics, supporting titration and annual medication reviews, optimising treatment and advising colleagues on the safe, effective and evidence-based use of medicines.

The post holder will also play an important role in developing the service, strengthening medicines governance and supporting shared-care arrangements across North East London. Through clinical leadership, education, audit and quality improvement, they will help us deliver a safe, accessible and patient-centred ADHD pathway that improves outcomes and reduces health inequalities.

Main duties of the job

• Deliver patient-centred Adult ADHD clinics as a non-medical prescriber, working within professional competence and the agreed clinical pathway.
• Assess, initiate, titrate and review ADHD medication, including annual reviews and transitions to shared care where appropriate.
• Undertake clinical risk assessments and contribute pharmaceutical expertise to complex case discussions and multidisciplinary team meetings.
• Provide expert advice on psychopharmacology and the safe, evidence-based and cost-effective use of ADHD medicines.
• Support the development and implementation of prescribing guidelines, clinical protocols, standard operating procedures and pharmaceutical care pathways.
• Contribute to medicines governance, clinical audit, service evaluation, research and quality improvement.
• Work collaboratively with primary care, pharmacy colleagues, commissioners and other internal and external partners to support joined-up care.
• Promote shared decision-making, provide clear medicines information and help service users achieve the best possible treatment outcomes.
• Deliver education and training to multidisciplinary and primary care colleagues.
• Provide clinical supervision, mentoring and, where required, line management to junior or trainee pharmacy staff.
• Support service development, co-production and initiatives that address unwarranted variation and health inequalities.

Detailed job description and main responsibilities
The Advanced Clinical Pharmacist will work as an integral member of the Adult ADHD multidisciplinary team, providing specialist pharmaceutical care across assessment, treatment, titration, follow-up and shared-care pathways.

The post holder will manage an agreed clinical caseload and deliver prescribing clinics within their scope of practice. They will assess patients, develop and review treatment plans, monitor the effectiveness and safety of medication and support adherence through shared decision-making. This will include recognising clinical priorities, completing appropriate risk assessments and escalating concerns or complex cases when required.

As a senior clinical practitioner, the post holder will provide expert advice on ADHD medicines and psychopharmacology, including the use of unlicensed medicines and areas where the evidence base is limited. They will critically appraise emerging evidence and apply relevant NICE guidance, MHRA alerts, formularies and medicines legislation to clinical practice.

The role will provide leadership for medicines optimisation and governance within the service. Responsibilities will include developing and reviewing clinical guidelines, prescribing protocols, standard operating procedures and care pathways; supporting safe repeat-prescribing arrangements; contributing to formulary applications; and identifying and reporting medication-related incidents and risks.

The post holder will play an active role in the continued development of the Adult ADHD Service. They will contribute to implementation of shared-care arrangements across North East London, help improve care across organisational boundaries and work with service users and colleagues to ensure that new ways of working are safe, inclusive and responsive to local need.

Audit, quality improvement and service evaluation will be central to the role. The post holder will use clinical, operational and prescribing information to identify opportunities to improve safety, quality, value and patient outcomes. They will be expected to participate in relevant projects and share learning through reports, presentations, professional networks and, where appropriate, conferences or publications.

The role requires close collaboration with service users and carers, pharmacists, medical staff, nurses, allied health professionals, operational leaders, general practices, primary care networks, commissioners and voluntary and community organisations. The post holder will communicate complex medicines information clearly and sensitively, helping colleagues and patients make informed decisions.

The post holder will support the learning and development of others by delivering training, providing clinical supervision and acting as a role model and mentor. Depending on service requirements, this may include line management of junior staff and supporting the development of non-medical prescribers.

The successful candidate will organise and prioritise their own workload, maintain accurate and timely clinical records and practise in accordance with General Pharmaceutical Council standards, legislation and Trust policies. The role will be based within Adult ADHD clinical services and will involve travel between NELFT sites, including sites across Barking and Dagenham, Havering and Redbridge. There may also be a requirement to contribute to dispensary, weekend or on-call pharmacy services in accordance with service need.

Starting with NELFT
NELFT place a great deal of importance on new starters being properly welcomed and inducted into the Trust. All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks which will include mandatory training, systems training and the allocation of equipment. As part of the process new starters will have the opportunity to also meet the executive team, senior managers, and attend a number of drop-in sessions focusing on engagement, health and wellbeing and key processes. The induction will be held at our head office in Goodmayes Hospital.

This post will be subject to a probationary period.

High Cost Area Supplement – Outer London
This post also attracts payment for High Cost Area Supplement of 15% of the basic salary (with a minimum of £4,714 to a maximum of £5,941 p.a. pro rata for part time).

High Cost Area Supplement – Fringe
This post also attracts payment for High Cost Area Supplement of 5% of the basic salary (with a minimum of £1,303 to a maximum of £2,198 p.a. pro rata for part time)
