Job summary
As the Lead Mental Health Practitioner, you will support the development of the mental health offer one of the South West Primary Care Networks. This will combine face to face assessment, triage, signposting and time limited psycho-social interventions with wider aspects, such as, linking in with key stakeholders, supervising members of the team, supporting recruitment and developments to meet the need of the local population.
As part of the mental health team in the Primary Care Network (PCN) you will share, with us, the Trust's vision and purpose, we will, together, deliver outstanding treatment and care that improves lives.
You will form part of the mental health offer and will be part of a team, reporting to the Community Matron.
Main duties of the job
Hampshire and Isle of Wight Healthcare NHS Foundation Trust provides joined-up mental and physical healthcare for around two million people across our communities. Our mental health services include community-based support and early intervention in psychosis (EIP) for both adults and young people, alongside a network of specialist inpatient wards covering forensic, learning disability, eating disorder and older person's care.
We deliver extensive physical health services too, from urgent community response teams helping frail and older patients remain safely at home, to hospitals at home teams providing acute-level care in familiar surroundings. Our neurological services offer rehabilitation and treatment for conditions including Multiple Sclerosis, Parkinson's Disease, Motor Neurone Disease, Head Injury, Cerebral Palsy and Stroke. Across Hampshire, our community hospitals provide inpatient rehabilitation as a step down from acute care, and our dedicated teams also staff Treetops Sexual Assault Referral Centre in Portsmouth, offering expert, compassionate support.

Main duties of the job
- To assume a leadership role and responsibility within the Mental Health Primary Care Team (MHPCT).
- To provide support, education and guidance of common mental health needs, their management and/or treatment, to colleagues across the multidisciplinary team, so they develop their skills and confidence.
- To provide line management to clinical roles in the MHPCT
- To provide inductions, clinical supervision, appraisal, and reviews, to clinical and practice staff within the MHPCT.
- To provide specialist clinical advice, consultation, and support to other care providers within primary, secondary, voluntary, and statutory services as required.
- To provide training to students monitoring and evaluating learning outcomes, providing feedback and assessing competencies to facilitate learning in practice.
- Participate in the recruitment and selection of staff in the clinical team.
- Act as the lead in Mental Health Clinician strategic forums between the Trust and GP Federations.
- Lead practice development initiatives within the team.
- Plan own workload and that of the evolving practice-based mental health team, managing competing demands to ensure care is delivered according to clinical priority and complies with defined indicators.
- Set quality standards for clinical work, monitor, and evaluate them. Report on the outcomes and propose actions plans to address any areas of concern, facilitating implementation and evaluation of agreed action plans.
