Clinical Summariser

Job summary
We are looking to recruit a dedicated and detail-focused Clinical Summariser to join our Practice team. This key role is responsible for reviewing, interpreting and accurately summarising clinical information from patient records, hospital correspondence and other healthcare documentation onto the Practice clinical system, ensuring records are complete, accurate and up to date.

As an integral member of the administrative team, the Clinical Summariser supports the delivery of safe and effective patient care by maintaining high-quality clinical records that assist clinicians in making informed decisions. Working in accordance with Practice policies, NHS guidance and information governance requirements, the postholder will contribute to accurate data recording, reporting and population health management.

The successful candidate will possess excellent organisational and communication skills, a meticulous attention to detail, and a strong commitment to confidentiality. They will be able to prioritise workloads effectively, demonstrate sound judgement and remain calm and professional while working within a busy healthcare environment. Previous experience of clinical summarising or working with medical records is essential.

About us
We are a large practice located in Warrington serving a population of around 12523 patients. The practice has a friendly, supportive team who work closely. This consists of 5 GP partners, 2 salaried GPs, 2 advanced nurse practitioners, 3 practice nurses, 2 pharmacy technicians, 2 healthcare assistants, 3 Managers and a team of 10 Administrators.

We are part of an active PCN and have good working relationships while sharing good practice with one another.

Job responsibilities

• Ensuring accurate and timely coding of clinical information onto patient records
• Summarising patient records in a clear, concise and clinically meaningful format
• Supporting clinicians with accessible, well-structured patient histories on Systmone
• Maintaining data quality standards in line with NHS and Practice requirements
• Contributing to improved patient outcomes through accurate record keeping and reporting
• Entering Read coded data into computerised medical records from letters/reports received from secondary care clinicians.

Person Specification
Experience
Essential

• Experience of administrative work in a healthcare or office setting. Experience handling confidential information.
• High level of accuracy and attention to detail.
• Knowledge of medical terminology.
• Experience of coding and summarising.

Desirable

• Experience within Primary Care. Experience of clinical coding and summarising Patient records and correspondence. Experience using Systmone.

Qualifications
Essential

• GCSE (or equivalent) in English (Grade C Level 4 or above)

Desirable

• Clinical Coding qualification (e.g. AMSPAR or equivalent)
