Home Support Matters - Triage Care Worker

What You'll Do
Play a critical role in supporting new customers integration, ensuring excellent standards of care from day one of a customer's journey with HSM.
Your responsibilities will include:
* Providing short‑term care and support, focusing on new customer integration.
* Building strong rapport with customers and their families from the outset.
* Identifying risks, safeguarding concerns and any required interventions.
* Communicating clearly with branch teams, field care supervisors and other professionals.
* Ensuring accurate, timely documentation that meets regulatory standards.
* Representing Home Support Matters with professionalism, empathy and expertise. What You'll Bring * Strong experience in care or support roles.
* Confidence in assessing needs and making informed decisions.
* Excellent communication and calm under pressure.
* Solid understanding of safeguarding and risk.
* A proactive, organised and professional approach.
* A Full UK Driving Licence and access to own vehicle with business insurance. Minimum availability must include: 07:00 - 13:30 AND/OR 16:00 - 22:00 With a requirement to work every other weekend or every saturday/every sunday.
