Job summary
We have a great opportunity to join our dietetic team at Ashford and St Peter's Hospital. We have a fixed-term vacancy for a 1.0wte Band 7 Specialist Community and PCN Dietitian for a 6month FTC, with likely extension to 11months. The post holder will be leading dietetic services for West Byfleet PCN working as part of a PCN multi-disciplinary team. They will also hold a clinical caseload as part of the Community Dietetics Team based at Ashford and St Peter's Hospitals. The Community Dietetics Team offers support to a range of patients in care homes and the community requiring nutrition support advice, enteral feeding, and those with complex needs who are unable to attend outpatient clinics.

Main duties of the job
As a practitioner with a high degree of autonomy you will take responsibility for planning and delivering your own caseload, acting as a source of expertise on dietetic/nutritional management to patients, carers, GPs, consultants as well as other health care professionals in both hospital and community settings.

The Community Dietetic Team form part of the Ashford and St Peter's Dietetic Department to provide nutrition and dietetic services to CSH Surrey Community Hospitals' in-patients, care home and community based patients in need of dietetic advice and review. This includes establishing and monitoring patients on enteral feeds in their own homes.

Prospective candidates should be efficient, enthusiastic and confident to work autonomously within a supportive team and their sphere of competence. They should have excellent communication skills, as the job will involve liaising with social care, clinical and community teams as well as patients and their carers.

You will need to be flexible in your approach to work. Experience in a range of clinical areas of dietetics as well as excellent organisational and prioritisation skills are essential to this role. The successful candidate should be able to demonstrate excellent team working skills. The post holder must have a valid UK licence and access to a car.

Job responsibilities
Specialist Community Dietitian (0.6WTE of role):
  • Act as a specialist for the local community managing the dietetic service provision to community Hospitals in-patients and staff and to community based patients in need of dietetic advice and review.
  • Take responsibility for the provision of dietetic services, including appropriate dietary counselling, lifestyle advice and nutrition education to clients, carers, families and health care professional in their own homes, Nursing Homes, Residential Homes and Locality hubs within NW Surrey.
  • Support the ongoing delivery of dietetic educational programmes on nutritional care for care home staff, GPs, other health care professionals working across NWS.
  • Lead on development of dietary protocols and guidelines for the service and contribute to the policies for the team as a whole.
  • Take an active role within the general functioning of the Dietetic Department, including contributing to student training and supporting junior team members as required
Specialist Primary Care Network Dietitian (0.4WTE of role)
Delivery of the PCN dietetic service will include initial clinical assessment, diagnosis, evaluation and treatment of patients nutritional care with a holistic view referring onto other agencies when required. The PCN Dietitian will:
  • Be responsible for leading the provision of dietetic services to a PCN.
  • Deliver dietetic clinics in the PCN and support integrated and collaborative provision of dietetics services alongside community and acute dietetic services in the locality.
  • Be responsible for monitoring effectiveness of the PCN dietetic service and work to sustain and improve this
  • Proactively seek engagement from local PCN leads to identify areas for service improvement, aiming to develop pathways of improved dietetic care to meet the health needs of the local population and population priorities in line with the NWS Alliance and Surrey Heartlands ICB.

Qualifications
Essential
  • 4 year degree in Dietetics (or equivalent qualification)
  • HCPC registration
  • Extensive post graduate training to Masters level or equivalent experience in practice at masters level in a subject relevant to at least of the core conditions (diabetes, frailty, obesity or functional bowel disorder)
  • Student supervisory skills course
Desirable
  • Membership of the British Dietetic Association
  • Completed or working towards Supplementary Prescribing rights by completing HCPC approved n-medical prescribing qualification
  • Teaching on degree (or higher level) / professional course
Experience
Essential
  • Significant experience in a range of clinical dietetic areas, including experience in managing patients in a specialist area
  • Previous experience at a senior level in community setting within the NHS environment
  • Experience of multidisciplinary team working and with partner organisations
  • Experience of contributing to and service delivery/evolving evidence led practice and delivery in a changing environment
  • Experience of conducting audits and/ or research
Desirable
  • Experience of working within a primary care setting
  • Experience of catering liaison and menu planning
Knowledge
Essential
  • Understanding of clinical governance
  • Evidence of continuing professional development e.g. advanced diploma in dietetic practice or equivalent short courses
