Job summary
We are excited to be increasing our Immunisation Outreach Team!

The service aims to increase the current levels of protection from preventable childhood infections among children and young people in Nottingham City. This will be achieved by delivering a population wide, evidence-based immunisation programme including a range of childhood Immunisations as part of the complete routine immunisation schedule (UKHSA) within the 0-19 population.

Nottingham City has significantly lower than average childhood immunisation rates across routine childhood and adolescent vaccinations as compared to the East Midlands and national rates.

The pilot is initially focusing on specified geographical areas and will work closely with 0-19 childrens services.

Previous immunisation experience not required, full training and support will be provided.

The service offer for targeted childhood catch up immunisations across Nottingham City through a range of community engagement projects and home visits. This post is fixed term until 31 March 2027.

Main duties of the job
This Band 5 Immunisation Community Public Health Nurse role involves delivering the national immunisation programme to children, families and adults across community settings such as schools, clinics and homes. The post holder provides safe, effective vaccinations in line with national guidelines, while identifying and responding to individual and population health needs. Key duties include maintaining accurate patient records, managing vaccine storage and cold chain processes, and recognising and responding to adverse reactions.

The role requires strong collaboration with multidisciplinary teams, partner agencies and local services to improve uptake and ensure equitable access, particularly within diverse communities. Nurses are expected to demonstrate high professional standards in line with the NMC Code, contribute to safeguarding, and support health promotion initiatives.

Working under supervision, the nurse must organise their workload effectively, travel across the city, and deliver high-quality, compassionate care while upholding organisational values. Continuous professional development, adherence to policies, and maintaining patient safety and confidentiality are essential aspects of the role.

Job Purpose

• To deliver the national immunisation programme provided to babies, children and adults in line with Department of Health and local Government guidance.
• To deliver an immunisation service within various settings in the community including schools, health centers, clinics and domiciliary settings.
• Respond to the unique immunisation needs of individuals and specific population groups.
• Anticipate, identify and manage adverse events following immunisation as appropriate to the practice setting.
• Act in accordance with professional, legal and ethical standards in all aspects of immunisation practice.
• To work in an inner city with a rich and varied ethnic, cultural and socioeconomic climate.
• Your practice must adhere to clinical guidelines, policies, and Standard Operating Procedures (SOP) both on a local and national level and you must demonstrate Nottingham CityCare Partnership CIC values and behaviors in all you do.

Dimensions

• To work under the direction and leadership of a Clinical Service Manager.
• Ability to travel across Nottingham City.
• Responsible for ensuring appropriate patient safety and that clinical governance indicators and measures are in place in accordance with organisational strategy, policies, and procedures.
• Promote a high-quality immunisation service to patients throughout Nottingham City.
• Deliver and expand immunisation provision as required in fulfilling a change to the national immunisation programme.
• Liaise with partner agencies to ensure effective and efficient delivery of immunisation meeting the needs of all patient groups e.g., general practices and schools.
• You are expected work in a cost-effective way ensuring responsible use of public funds.
• You will undertake mandatory, and role specific training as required.
• You will participate in management, safeguarding supervision and annual appraisal.
• The core service hours are 8.30am to 5pm, Monday to Friday excluding bank holidays. There is an expectation that you will work outside of these hours, holding community clinics until 8pm. Start and finish times will vary accordingly

Key Responsibilities
Quality and performance

• To be flexible as required in supporting service delivery across the whole City.
• To maintain contemporaneous healthcare records using SystmOne in line with National and Nottingham CityCares policies and procedures.
• Work using own initiative within the limitations of the role; always promoting high quality care and Nottingham CityCares values and behaviours.
• Participate in public health incentives as required.
• Responsible for the safe use, transportation, ordering and storage of all equipment relevant to the post. This will range from basic items such as sharps boxes and clinical waste bags to syringes, needles, and vaccines.
• The post holder will be responsible for ordering vaccines and ensuring pharmacy refrigerators and pharmacy porters are used in accordance with cold chain guidelines. Maintaining vaccine cold chain in community settings.
• The post holder will be responsible for ensuring appropriate equipment is at hand for the treatment of anaphylaxis.

Professional

• To work collaboratively as a member of the wider 0-19 Public Health Nursing Service within Nottingham CityCare and assist with the development of the immunisation team and related workplans.
• To act in accordance with The NMC Code and ensure that the highest standard of professional conduct is maintained.
• The post holder is required to take responsibility for their own professional development, NMC revalidation and to identify own continuing professional development needs through annual appraisal and maintain a personal development plan.
• The post holder is required to comply with all Statutory Policies and Procedures of Nottingham CityCare e.g. Health and Safety, promoting attendance at work and data protection.
• Work in accordance with Nottingham City Local Safeguarding Childrens Board procedures. If required attend/contribute to safeguarding procedures/meetings.
• Participate in supervision e.g. clinical, managerial in line with organisational guidelines, and take all opportunities to reflect on practice.
• To demonstrate leadership and contribute to the appraisal, development and monitoring of performance of team members induct and mentor staff and students.
• Support the team and organisation to provide a learning environment and experience for new staff and students.
• Provide and receive routine information, orally, in writing or electronically in line with Nottingham CityCare policy and abiding by legal requirements.
• Complete all appropriate records/documents in a clear, accurate and timely way, participating in reporting processes and audit processes as necessary.
• The post holder will be expected to use IT equipment to record copies of data and work to the guidance of Patient Specific Direction (PSD) and Patient Group Direction (PGD).
• Ability to organise and prioritise own workload under the supervision of senior members of the team.
• The post holder will be responsible for the maintenance and safe and efficient use of equipment in accordance with local policy: Non-Clinical equipment, e.g. Health Promotion Materials, IT equipment. Clinical Equipment e.g., cold storage/shock packs/vaccines/sharps etc.
• Incident reporting including adverse event system e.g., DATIX as required.
• Manage situations where assertiveness is required when supporting the best interests of the client, e.g. clients with mental health problems, disabilities.
• Demonstrate empathy, compassion, and ability to reassure.
• Be sensitive to the needs of service users who may have communication problems and implement to accessible information standard. Where English is not the first language or understanding is limited, identify appropriate resources to optimise understanding for clients.
• Work with interpreting services as required.
• Always maintain confidentiality.
• Use of mobile phones, internet, emails, and intranet will be required for communication.
• Maintain an accurate electronic diary in accordance with lone working policy and adhere to mobile working guidance.
• Contribute to research, surveys or audits as required by the service.
• At all times ensure that own actions support and promote equality, diversity and the rights of patients, the public and colleagues within the health care environment.

Knowledge, training and experience

• Preferred experience of delivering immunisation programmes and understanding of health protection.
• Up to date knowledge on current immunisation issues and national priorities.
• To attend mandatory and essential and role specific training sessions as defined by Nottingham CityCare Partnership.
• To have an understanding of CityCare policies and procedures related to your clinical role.
• To receive annual review and personal development planning to promote achievement of individual, team and organisational objectives.
• Complete a comprehensive competency package through observation, reflection and supervised practice within the first 6 months of being in post.

Person Specification
Qualifications
Essential

• First level registered Nurse/Midwife with current NMC registration.
• Evidence of continuous professional development.

Desirable

• Training that meets the National Minimum Standards and Core Curriculum for Immunisation Training, Health Education England E-Learning for Healthcare.
• Student Nurse Supervisor Assessor trained.

Experience
Essential

• Experience of working in community/primary care setting

Desirable

• Experience of working in a skill mix team and carrying out delegated work.
• Experience of using electronic record keeping systems and contributing to updating health records.
• Evidence of working with diverse/ disadvantaged groups.
• Evidence of working with Safeguarding children / adults and vulnerable adults.
• Experience liaising with other services e.g., public health, child health, pharmacy, schools and GPs.
• Post registration experience of immunisation administration.
• Previous lone working experience.
